SIP Advanced Application​

SIP trunking for your voice system.

"Company Name" is the legal entity under which the service will be registered. In this form, "you", "I", or "us" refers to that Company. "ACN/ABN" is your Australian Company Number or Australian Business Number. "Trading/Business Name" is not a legal entity but the name under which your business trades.

Contact Details

The Contact Name is the person representing you for billing and contract administration. You must notify us of any changes to your contact details.

Technical Contact Details

The Technical Contact Name is the person representing you for the technical aspects of the service — your authorised representative for the service.

A $500 (ex GST) activation fee applies per connected SIP Advanced service. This fee is waived if you take up a SIP Advanced plan with a minimum contract term of 36 months.

Included in your SIP Advanced plan:Local calls National long distance (STD) calls Calls from fixed lines to mobiles within Australia Voice carriage access Unlimited calls of the types listed aboveCalls to 13/1300 numbers, premium numbers, and international calls are charged separately at the rates set out in Teltech Connect's Customer Terms.Changing Plans and Early Termination Charge (ETC)If either party cancels your SIP Advanced service while more than 6 months remain in your minimum term, an ETC applies equal to 30% of your monthly service charges for the month of cancellation, multiplied by the number of months (or part thereof) remaining in your minimum term. You will also need to separately cancel any other services used in conjunction with your SIP Advanced service.

To take up SIP Advanced, you must also have:

• An accredited PBX configured to our specifications (if no ISDN IAD is required). We will confirm whether your voice system is accredited when you apply.
• QoS-capable routers and switches, with QoS configured on your WAN and LAN.
• Sufficient bandwidth to support the required number of channels.
• Where using Internet, NBN, or any other access service, sufficient bandwidth to support the required channels.

You may select one or more optional feature packages for your SIP Advanced service. Please indicate the quantity of each package you are applying for. One Business Continuity Pack must be allocated per SIP Advanced service, and one Real Time Billing service must be applied per SIP Advanced service.

About the Optional Feature Packages

• Business Continuity Pack: automatically detects when the network loses connectivity to your equipment. Calls are then re-routed and handled via another trunk group or specified number, without you needing to call us to arrange temporary call diversions.
• Mobility Pack — Remote Office: lets you make and receive calls as if at your desk. Incoming calls are automatically rerouted to your remote office number, and outgoing calls appear to originate from your desk number, without consuming bandwidth on your local site.
• Mobility Pack — SimRing: rings up to 10 end points simultaneously so calls are answered quickly, without consuming additional capacity on your local PBX.
• Portal access - Allows for access to the backend customer portal that allows view access on the account, billing details usage.

As part of your SIP Advanced service you will receive an administrator account for the Teltech Connect self-service portal, which lets administrators invite, modify, and remove associated services.

Local Number Portability (Porting In)If you would like us to port in numbers from another provider to your SIP Advanced service, numbers must be ported in blocks of at least 100 contiguous numbers (exceptions may apply if porting from a legacy custom voice network service).By providing the details below, you authorise us to port in the listed numbers to your SIP Advanced service.

Migrating Numbers from an Existing Teltech Connect ServiceIf you would like us to migrate numbers from an existing Teltech Connect service to your SIP Advanced service, numbers must be migrated in blocks of at least 100 contiguous numbers (exceptions may apply for legacy custom voice network services), and numbers cannot be moved outside our Standard Zoning Units.

Teltech Connect's Customer Terms means the Standard Form of Agreement formulated by us for the purposes of Part 23 of the Telecommunications Act 1997 (Cth), as amended by us from time to time. I apply for the service components in this Application Form and acknowledge that, if accepted, the service will be provided on the terms set out in this Application Form, its attached schedules, and Teltech Connect's Customer Terms. If there is any inconsistency between this Application Form and Teltech Connect's Customer Terms, the Customer Terms will apply to the extent of that inconsistency. I acknowledge that I have received, or had the opportunity to review, a copy of Teltech Connect's Customer Terms.Privacy ConsentI agree, and will ensure that any related bodies corporate receiving services connected with this Application Form, and our representatives, are aware, that Teltech Connect may:

• collect, use, and disclose information about each of us (including for marketing purposes) in accordance with Teltech Connect's Privacy Statement, as amended from time to time; and
• give to, seek, and obtain from a credit reporting agency information (including personal and credit information) about each of us to assess creditworthiness.
